Our Youth, Our Future Survey To Be Conducted at Hoopa Valley High School
On April 9th students in 9th – 12th grades will begin participating in the Our Youth, Our Future survey in cooperation with the Tri-Ethnic Center for Prevention Research at Colorado State University. The funding for the administration of the survey comes from a federal grant awarded to the Tri-Ethnic Center, which allows the survey to be conducted at no cost to the school. Participation in this survey is voluntary. Parents who wish to withdraw their child from participating may do so by contacting the attendance secretary, Tashone Rice, at 530-625-5600 ext. 1016. In addition, a copy of the survey is available at the school for parents who would like to review it. Hoopa Valley High School believes that students’ participation in this survey will assist the school in planning, funding and evaluating drug and alcohol prevention programs.
I will be the contact person for all PK-12 students. The students who return their consent forms will be scheduled during the school hours and will either be transported to the High School by the District or will be escorted to the High School from surrounding campuses.
We will also be including pre-school age children that will enter school as a Kindergartener for the 2018-2019 school year. The children must be able to read a picture chart. If you have a pre-school age child that qualifies and you would like to schedule a vision exam contact:

In order for your child to take advantage of this great program, they must return the enclosed permission slip. Our goal is to service 1200 children. We need to reach this goal for the OneSight Program to come and offer this wonderful opportunity. Please complete the permission slip and return it to your child’s teacher immediately, so we can finalize our appointment schedule.
The One Sight Program will also be offering free vision exams and glasses to 400 adults. If you would like to schedule a vision exam for an adult, please contact Paula Gray at K’ima:w Medical Center (530) 625-4261 to schedule an appointment.
